How many units for masseter Botox?

Why nobody can give you a number over the phone

This is the most-searched question in the masseter category, and every credible answer is a range. Here’s why that isn’t a dodge.

The masseter is the thick muscle running from your cheekbone down to the angle of your jaw — it’s the one that bulges when you clench. It is also, pound for pound, one of the strongest muscles in the human body. And its size varies enormously between people. A 5’2” woman who doesn’t grind her teeth and a 6’2” man who has clenched through a decade of stress have masseters that are not remotely the same object. Dosing them identically would be malpractice-adjacent.

So the number depends on three things: how big your masseter is, what you’re trying to accomplish, and which product we’re using.

Typical ranges — with the caveats attached

With the standard caveat that this is educational and not a dose prescription for you specifically, published ranges and common clinical practice for masseter neurotoxin land roughly here:

• Smaller masseters / conservative first pass — around 20–25 units per side
• Average masseters — around 25–30 units per side
• Large, heavily developed masseters (long-term clenchers, larger frames) — 30–50 units per side

Which means a full masseter treatment commonly totals somewhere in the 40–60 unit neighborhood, and can run higher. Note the “per side” — masseter is a bilateral treatment, and people quoting themselves a total based on a per-side number are a big source of sticker confusion.

One more wrinkle that matters, and it’s the reason unit talk gets muddy: units are not interchangeable across products. A unit of one neurotoxin is not equivalent to a unit of another. Jaw slimming vs. TMJ relief — different goals, different plans

People arrive at masseter treatment from two directions, and they aren’t the same treatment even though they use the same muscle.

TMJ and clenching. You grind at night, you wake with a tight jaw, you get tension headaches, your dentist has mentioned your enamel. The goal is reducing the force of the clench. This tends to be about function, and people often notice the muscle relaxing before they notice their face changing.

Jaw slimming. You want a softer, more tapered lower face. Chronic clenching hypertrophies the masseter — it’s resistance training you didn’t sign up for — and the visible squareness is the result. The goal here is atrophy over time, which is why this one is a program, not an appointment.

The timeline nobody tells you about

This is where masseter treatment differs most from a forehead or a glabella, and where expectations go wrong.

TMJ relief tends to show up in the first couple of weeks, once the muscle’s contraction force drops.

Visible slimming takes considerably longer — commonly two to three months after the first session, and often meaningfully more after the second and third. You’re not relaxing a wrinkle; you’re waiting for a large muscle to lose bulk from disuse. That takes time. Most people running masseter for aesthetic slimming are looking at two to three sessions over six to twelve months before they see the shape they came in for, and maintenance after that.

If someone tells you your jawline will change in a week, they are describing a different treatment.

Individual results vary, and some people simply respond less than others.

What to ask at your consult

Bring these. A good injector will welcome them.

1. Where exactly are you injecting, and how are you avoiding the risorius? Placement matters — inject too far forward or too superficially and you can affect the smile. This is an anatomy job.
2. How many units, per side, and why that number for my muscle?
3. Am I here for function or for shape? The plans differ.
4. What happens to my chewing? Most people notice mild fatigue with tough foods for a few weeks. It settles.
5. What’s the maintenance cadence after the initial series?
