What's the actual mechanical difference between the two?

Receptor targets. Semaglutide acts on the GLP-1 receptor. Tirzepatide acts on GLP-1 and GIP — two incretin pathways rather than one. More targets is not automatically better *for you*; it means a different pharmacology with a different profile, and which one suits you is a clinical question your physician answers with your history in hand — not a ranking.

Why does the starting dose matter so much?

Because side effects are overwhelmingly a titration story. Semaglutide starts at 0.25mg weekly and Tirzepatide at 2.5mg — both well below the working range, on purpose. Climbing that ladder slowly is what separates a manageable few weeks from a miserable ones. A program that rushes you up the dose to show a fast number on the scale is optimizing for the wrong thing.

Is the more expensive one better?

Not inherently, and this is the assumption we most want to dislodge. Plenty of people do extremely well on Semaglutide at $199/month and would gain nothing from spending $100 more a month. The price gap reflects the medication's cost and complexity, not a quality tier. Start where your clinical picture points, not at the top of the menu.

What are the side effects, really?

Nausea, appetite changes, and GI effects are common — most pronounced during titration and in the days after a dose increase. Most people find they settle. Some don't tolerate a given medication and we change course. This is a real class of drug with real effects, which is precisely why physician supervision is part of the price rather than an upsell. Individual results vary.

Self-inject or in-office?

Most people self-inject. It's weekly, the needle is small, and it stops being a thing within a month. In-office injections are available for an additional $100/month if you'd rather not — some people simply don't want a needle in their fridge, and that's a perfectly good reason.

What about muscle loss?

It's the most under-discussed risk in this entire category. Aggressive appetite suppression without adequate protein and resistance training costs you lean mass alongside fat, and lean mass is the thing your metabolism runs on. You’ve probably arrived here already knowing the names. Semaglutide. Tirzepatide. What nobody explains clearly is how they differ — so here it is, without the marketing.

The mechanism, briefly

Your gut releases hormones called incretins when you eat. They signal fullness, slow gastric emptying, and influence how your body handles glucose. GLP-1 is one of those hormones. GIP is another. Glucagon is a related signal operating on a different axis.

Everything in this drug class works by acting on those pathways. The difference between the two medications is simply how many of those pathways each one touches:

  • Semaglutide — GLP-1
  • Tirzepatide — GLP-1 + GIP

That’s the actual technical distinction, and it’s usually buried under adjectives.

The trap in reading that list

More receptors looks like more better. Resist that.

More targets means different pharmacology, not a straight upgrade. A different profile, different response patterns, a different set of trade-offs. Which one suits you depends on your history, your tolerance, and your goals — and that’s a clinical determination, not a shopping decision made from a webpage.

Plenty of people do very well on Semaglutide at $199/month and would gain nothing whatsoever from spending an extra $100 a month for a broader mechanism they don’t need. Read the starting doses again

Both are well below the working range, deliberately. That’s the titration ladder, and it is the single most important thing to understand about this class of drug — because side effects are almost entirely a titration story.

Nausea and GI effects cluster around dose increases. Climb slowly and most people find they settle. Climb fast chasing an early number on the scale and you’ll have a miserable month and possibly quit a medication that would have worked.

This is where physician supervision earns its place. A program with an incentive to show you a fast result has an incentive to push the ladder. Ours doesn’t.

The risk nobody advertises

Muscle.

GLP-1s suppress appetite powerfully. If you eat far less without protecting protein intake and doing resistance training, you lose lean mass alongside fat — and lean mass is what your metabolism runs on. You end up lighter, weaker, and metabolically worse positioned than when you started, which is a genuinely bad outcome dressed up as a good number.

This is the strongest argument against a subscription app that ships you a pen and calls it a program. It’s why coaching sits inside ours, and why we’d rather talk you out of a fast result than sell you one.

And the question before all of these

Is a GLP-1 even the right tool for you?

Sometimes the honest answer is no. A thyroid issue, hormones, sleep — things that no GLP-1 addresses and that will quietly undermine one.
